CORI Form

Massachusetts requires all schools to conduct criminal background checks (CORI) every three years on current and prospective employees. Volunteers, school transportation providers, and others who may have direct and unmonitored contact with children, need to complete a CORI every year. Bridgewater-Raynham RSD has been certified by the Criminal History Systems Board to access such information, and all parents/guardians who wish to volunteer or chaperone school activities are required to complete a CORI Authorization Form.

Since it may take several days to conduct and review such a check, parents/guardians who have not had a CORI check through Bridgewater-Raynham RSD during the year may wish to do so at the beginning of the school year. With your completed CORI check kept in our confidential file, you will then be able to volunteer or chaperone any time during the year. CORI checks require proof of identity in the form of a government issued photographic identification such as a driver’s license.

Once the information is filled in, you will need to print the form and bring it along with your government issued identification to Central Office or your child’s school. CORI forms are available online, in each school office and at the central administration office.

NOTE: An applicant may remotely complete the CORI form by having it notarized (on the 1st page of the form). This means the individual would present the signed CORI form with government issued identification to a Notary prior to submitting it to the school to be processed.
